The tax exemptions provided to non-profit hospitals and health systems had an estimated value of nearly $28 billion in 2020. The total reflects estimated federal, state, and local taxes; increases in charitable contributions; and decreases in bond interest rate payments. About 60% of community hospitals nationwide hold federal tax-exempt status. In exchange, the hospitals provide charity care (free or discounted services) and other community benefits, including unreimbursed expenses related to Medicaid and health professional education, and certain subsidized health services.
