Telehealth Intervention By Pharmacists Collaboratively Enhances Hypertension Management & Outcomes
Blood pressure control can improve significantly when individuals participate in telehealth visits integrating home blood pressure monitoring (HBPM) readings with a pharmacist between primary care visits, according to a recent study.
Researchers conducted a retrospective observational cohort study within the CAMP clinic at UNC Health. They focused on the Healthcare Effectiveness Data and Information Set (HEDIS) Controlling High Blood Pressure (CBP) measure, an evaluation of the proportion of adults ages 18 to 85 with hypertension whose blood pressure is controlled, which is defined as being less than 140/90 mm Hg during the measurement year.
