In January 2012, the Centers for Medicare & Medicaid Services (CMS) published an update to the 2011 edition of the chronic conditions chartbook. The chartbook provides an overview of chronic conditions among Medicare beneficiaries, highlights the prevalence of chronic conditions among Medicare beneficiaries, and documents the impact of chronic conditions on Medicare service utilization and spending. CMS selected 15 common chronic conditions that are available in the CMS Chronic Condition Warehouse (CCW) research files that also correspond with the list of chronic conditions used to define multiple chronic conditions by the Department of Health and Human Services . . .
