Maine State Innovation Model: The Operations Plan for Sustainable Health Care Reform

In August 2013, the Maine Department of Health and Human Services released its operations plan for the Maine State Innovation Model (SIM). The plan is to serve as a guidebook to help Maine achieve its Triple Aim objectives and transform health care.
