Gettysburg, PA. (August 1, 2023) – OPEN MINDS announced today a renewed collaborative partnership with Alera Health for the 2023-2024 year. Alera Health specializes in building and managing Clinically Integrated Networks (CINs) for patients suffering from primary and secondary behavioral health conditions. These multi-specialty Integrated Systems of Care (ISCs) are designed to promote insight and communication between providers of primary, behavioral health, hospital, and social support agencies under value-based agreements. The Alera Health model, known as “ONEcare“, brings providers together to share data, insights, technology, and best practices to improve health outcomes and lower cost for the most complex polychronic patients.
