NEW YORK, Ny. (February 16, 2021) — AsOne Healthcare IPA (AsOne), a New York City-based integrated behavioral health and primary care provider network, has launched the first-of-its-kind IT and data solution to facilitate population health management and improve patient outcomes. One aspect that is particularly unique is the ability to view aggregated outcomes by groups, thereby supporting the implementation of intergenerational and family-focused models of care as well as new outcomes-based reimbursement mechanisms.
The new platform, called LEADS, was developed by Netsmart Technologies, Inc (Netsmart) and implemented by Informd. LEADS is designed uniquely, in collaboration with the AsOne network, to aggregate data from electronic health records and other sources from all 19 AsOne network providers and house it in a centralized data platform, enabling an integrated system of health care delivery. The name LEADS serves as a mnemonic to outline the platform’s functionality and innovative features, which include:
- Linking providers’ databases and EHRs within a centralized aggregated warehouse
- Facilitating exchange of data to improve outcomes through enhanced clinical integration
- Enabling data analytics, reporting, and dashboards to provide key insights and impact performance
- Driving clinical outcomes with real-time information and a 360° view of an individual and family
- Supporting family functionality to enable grouping of interrelated patients and clients
“We are thrilled to be working with AsOne as they operationalize this transformative population health management program” said Mike Valentine, Netsmart CEO. “By leveraging the Netsmart CareFabric platform to drive this program in an EHR agnostic manner, AsOne will be able to bring data visualization, risk stratification and care insights to the providers in their network. It is exciting to be partnered with a group leveraging technology in such a proactive and innovative manner within today’s shifting healthcare climate.”
AsOne was established in 2018 to create an evidence-based health care treatment system comprised of community-based organizations who are providers of primary care, mental health care, substance abuse treatment, care management, and an array of social supports targeted to children, adults, and families, that collectively act “as one” in delivering whole-person care in a family-based approach.
“It is a privilege to help bring AsOne’s vision to life” said Jawad Sartaj, CEO of Informd. “The ambitions of AsOne –specifically the complex family treatment model – is only made possible through a digital platform which provides the right information to the right caregiver at the right time. LEADS aggregates various datasets to create an enriched patient record and provides AsOne’s network with decision support capabilities on nearly 250,000 Medicaid lives. We look forward to our continued support of AsOne and the expansion of the platform in service of at-risk individuals and families.”
As a key component of this overall vision, AsOne has developed a new “Complex Families Treatment Model” that leverages an interdisciplinary care team to deliver a clinical therapeutic intervention to families with complex health and social needs. This model seeks to interrupt the perpetuation of serious chronic health conditions, both physical and behavioral, that are passed down through generations of families and communities.
“We know that helping people achieve good health depends on addressing the physical and behavioral needs of their families, but the fragmentation of health care data undermines this proven whole health approach,” said David Woodlock, President and CEO of the Institute for Community Living and founding Board member of AsOne. “LEADS is an innovative IT solution that will make it easier for the AsOne network to help thousands of families with complex health needs get better.”
AsOne is a health care Independent Practice Association (IPA) that exists to facilitate value-based payment contracts with payers, including managed care organizations, on behalf of its clinically-integrated provider network, to provide health care and related social services. Currently, AsOne’s 19 network providers serve over 250,000 children, adults, and families with complex physical, mental, and social needs throughout the five boroughs of New York City.
AsOne is currently funded through the NYS Behavioral Health Value-Based Payment Readiness Grant, a multi-year initiative. The purpose of this initiative is to help increase provider readiness to operate in a value-based payment (VBP) environment.
