NEW YORK, Ny. (February 16, 2021) — AsOne Healthcare IPA (AsOne), a New York City-based integrated behavioral health and primary care provider network, has launched the first-of-its-kind IT and data solution to facilitate population health management and improve patient outcomes. One aspect that is particularly unique is the ability to view aggregated outcomes by groups, thereby supporting the implementation of intergenerational and family-focused models of care as well as new outcomes-based reimbursement mechanisms.

The new platform, called LEADS, was developed by Netsmart Technologies, Inc (Netsmart) and implemented by Informd. LEADS is designed uniquely, in collaboration with the AsOne network, to aggregate data from electronic health records and other sources from all 19 AsOne network providers and house it in a centralized data platform, enabling an integrated system of health care delivery. The name LEADS serves as a mnemonic to outline the platform’s functionality and innovative features, which include:

  • Linking providers’ databases and EHRs within a centralized aggregated warehouse
  • Facilitating exchange of data to improve outcomes through enhanced clinical integration
  • Enabling data analytics, reporting, and dashboards to provide key insights and impact performance
  • Driving clinical outcomes with real-time information and a 360° view of an individual and family
  • Supporting family functionality to enable grouping of interrelated patients and clients

AsOne was established in 2018 to create an evidence-based health care treatment system comprised of community-based organizations who are providers of primary care, mental health care, substance abuse treatment, care management, and an array of social supports targeted to children, adults, and families, that collectively act “as one” in delivering whole-person care in a family-based approach.

As a key component of this overall vision, AsOne has developed a new “Complex Families Treatment Model” that leverages an interdisciplinary care team to deliver a clinical therapeutic intervention to families with complex health and social needs. This model seeks to interrupt the perpetuation of serious chronic health conditions, both physical and behavioral, that are passed down through generations of families and communities.
