OPEN MINDS Executive Roundtable: Strategies For Hybrid Care For Diverse Populations, The MyCHN Case Study, To Be Held on August 21, 2025
Gettysburg, PA (August 17, 2025)—OPEN MINDS invites leading health care executives to join the Executive Roundtable, Strategies For Hybrid Care For Diverse Populations, The MyCHN Case Study, to be held on August 21, 2025, at 1:00 PM ET. This one-hour webinar will showcase how MyCHN is delivering integrated, whole person care.
The session will feature MyCHN leadership—Yvette Poindexter, M.D., MBA, Chief Medical and Operations Officer; Penny Pabst, Chief Administrative Officer; and Nydia Garcia, Quality Director—as they share their strategies for delivering hybrid care that meets the complex needs of diverse consumer populations.
MyCHN’s model combines telehealth visits, remote patient monitoring with connected medical devices, and AI-powered mental health support, creating a seamless consumer experience. From the first point of contact, consumers have access to medical, dental, behavioral health, and lab services, ensuring coordinated care across disciplines. If needed, behavioral health consumers receive navigation to same- or next-day primary care. Primary care consumers are screened for depression, anxiety, tobacco, alcohol, and drug use and referred to behavioral health services as appropriate.
“For executive teams of traditional specialty care provider organizations, moving to whole person care models means finding a sustainable service line approach their organizations are capable of delivering,” said Monica E. Oss, Chief Executive Officer of OPEN MINDS. “To keep on top of the market, health and human service executives need to have a process for integrating technology innovations into their whole person care models.”
Executive attendees of this session will:
- Review operational integration pathways and parameters initiating simultaneous integration (primary care, behavioral health integration, dental hygiene).
- Review health care outcomes data for the behavioral health integrated patient population compared to overall health center data.
- Review dental best practices for integration into primary care and behavioral health.
