Primary care practices certified as health care homes (HCHs) had higher performance on a composite measure of five important outcome measures of diabetes care, compared to uncertified practices. Scores for composite optimal diabetes care averaged 46.8% for certified practices, and 43.2% for uncertified practices. The five domains of practice systems include:

Access: continuous access and communications with consumers and family
Registry: an electronically searchable registry to identify care gaps and manage services
Coordination of care: care coordination for consumer- and family-centered care
