Social resource connection platforms that report closed loop referrals (CLRs) use the metric to understand how many referrals can be tracked to determine referral outcome. However, the closed loop rate should not be used to measure how often the referrals result in the consumer actually receiving services to address health-related social needs. The potential for confuson about what CLR means was noted by researchers from Duke Health who analyzed the service connection rate through NCCARE360, the first statewide social resource connection platform for placing and tracking referrals.
