Consistent Home Health Staffing Reduces Hospital Readmission For Those With Dementia
People with dementia receiving home health care following a hospitalization were more likely to avoid rehospitalization when they had continuity of home health nurses during the period following discharge. Those who had low or moderate continuity of care were 33% and 30% respectively more likely to be rehospitalized than those who received high continuity of care.
