Maine Will Select A Contractor To Improve The Quality Of FQHC Chronic Disease Management Services

The Maine Department of Health and Human Services, Maine Center for Disease Control and Prevention (Maine CDC) will soon select a contractor to ensure that chronic disease management services are delivered by Federally Qualified Health Centers (FQHCs) in the state. The state issued a request for proposals (RFP 202107105) on July 24, 2021, with proposals due by August 19, 2021. The initial contract term runs from October 1, 2021, through September 30, 2023, followed by one three-year renewal option through June 30, 2025.
