Illumination Foundation’s Model For Successfully Integrating Social Determinants Of Health Into Broader Health Interventions Featured In Upcoming OPEN MINDS Executive Roundtable

Illumination Foundation CEO, Paul Leon, to join OPEN MINDS and present highly-anticipated case study at virtual event scheduled for February 24, 2022

Gettysburg, Pa. (January 23, 2022) – OPEN MINDS will host a Circle Elite Executive Roundtable on Thursday, February 24, at 1:00 pm ET featuring Illumination Foundation—A Case Study On Integrated Social Support. Paul Leon, Chief Executive Officer, will discuss how Illumination Foundation (IF) approaches the coordination of social determinants of health (SDOH) into broader health interventions, funding opportunities for SDOH programs, and best practices for treatment planning. OPEN MINDS Vice President Richard Louis, III will join Leon in the session which will be moderated by OPEN MINDS Senior Associate Sharon Hicks, MSW, MBA.

With many value-based arrangements and health plans focusing on managing care for underserved populations and engaging with SDOH programs, the outcomes achieved from reducing homelessness have clearly demonstrated the effectiveness of these interventions in reducing health care expenditures. Other SDOH programs are being funded through the advent of numerous 1115 waivers and Home and Community-Based Waiver Service (HCBS) programs. Mr. Leon will discuss the programs developed by IF, describe their 3 Spokes Model, and answer questions from our executive audience about how linking people with housing, helps to improve health outcomes for homeless adults with serious mental illness and substance use disorders.

CEO & Founder of the Illumination Foundation, Paul Leon was instrumental in the conception and development of the Comprehensive Health Assessment Team for the homeless (CHAT-H) and the Mobile Unit Teams (MUT), bringing targeted services to the underserved at the individual, family, community, and systems levels of practice while a County of Orange Health Care Agency Public Health Nurse and pioneered IF’s Recuperative Care Program. Paul consulted on innovative funding methods for recuperative care with the Health Resources and Services Administration (HRSA) across the country and in Puerto Rico. Paul has gained recognition for IF with several awards, including a place on the 10 AARP Well-Being Champions and in 2019, the AARP Purpose Prize Award. In addition to serving on several advisory committees for organizations, Paul mentors and conducts classes for students at Cal-State Long Beach, Cal-State Fullerton, Biola University, Concordia University, USC School of Social Work, and UCI’s School of Medicine. He also consults with CHOC’s Residency Program and the Kaiser Family Resident Program and served on the Board of Directors for the National Health Care for the Homeless Council (NHCHC).
