Access Services & Tower Health Present Consumer Street Outreach Model At September 15 OPEN MINDS Circle Elite Executive Roundtable
Gettysburg, Pa. (August 21, 2022) Debra Curtin, MS, Senior Director for Behavioral Health, and Mark Boorse, MHS, Director of Program Development with Access Services, Casey Fenoglio, MPH, CHES, Community Wellness Program Manager for Pottstown Hospital, Tower Health, and Haleh Van Vliet, M.D., Emergency Medicine Physician for Tower Health, will present Bringing Care To Consumers Through Street Outreach — The Access Services Case Study at the September 15 OPEN MINDS Circle Elite Executive Roundtable. The homeless population is one of the costliest cohorts and the least likely to follow through with treatment plans and health care program initiatives. To address these consumers, Access Services and Tower Health joined forces to reach consumers in the street. During this one-hour event, our speakers will share their model for reaching this disadvantaged population, their financial model, and how to overcome the systemic issues driving these consumers from treatment.
“Consumers suffering from homelessness are one of the most disadvantaged groups within the health care system,” noted Monica E. Oss, chief executive officer of OPEN MINDS. “It is imperative that organizations employ strategies to reach this population, which improves their personal health outcomes, reduces inappropriate emergency department utilization, drives down organizational costs, and improves community health overall.”
In this roundtable, our panel will share their service delivery model and approach to improving access and engagement in care for the homeless population. The session will cover how the collaboration between Access Services and Tower Health has led to improved compliance, more appropriate care venue usage, and health care savings. Attendees will learn about establishing partnerships with external organizations to serve as stakeholders and ensure program fidelity, identifying and addressing any barriers to receiving care, and helping consumers maintain financial independence, and/or hold stable housing. The speakers will share first-hand knowledge on establishing sustainable funding for investment in the community and how to continuously innovate to ensure operations are up to date as organization’s move towards new cultural goals and health care systems.

Debra Curtin, MS, is the Senior Director for Behavioral Health at Access Services, focusing on fundraising and advocacy to support new housing and homeless programs. Prior to this role, she was the Director of the Homeless Street Outreach Program and was involved in the program’s initial startup and implementation as well as the design and launch of the Street Medicine Program in Pottstown, Montgomery County, PA. Ms. Curtin’s experience includes work in mental health, housing and homelessness, and children and families service systems. Ms. Curtin is passionate about integrated health, community service and collaboration across service systems.
Mark Boorse, MHS, is the Director of Program Development at Access Services. In this role, he has been instrumental in the design and startup of the Homeless Outreach Program. He was also integral to the design and launch of the pilot Street Medicine Program. As part of the Street Medicine team, Mr. Boorse fills the role of Street Outreach Guide, bridging the gap between the homeless community and the medical providers on the team. He is passionate about helping marginalized community members gain access to care that promotes whole-person health.
Casey Fenoglio, MPH, CHES, is the Community Wellness Program Manager at Pottstown Hospital for Tower Health and also serves as the Vice President of the Board of Directors for TriCounty Community Network. Prior to joining Pottstown Hospital in 2019, Ms. Fenoglio’s public health experience ranged from university research to school wellness to health education and prevention for a national non-profit organization. In her current role, she works to identify and address the community health needs in Pottstown Hospital’s service area.
Haleh Van Vliet, M.D., is a board-certified emergency medicine physician with Tower Health Medical Group. She completed her undergraduate studies in Neuroscience at Vanderbilt University before obtaining her medical degree from the University of Rochester School of Medicine with training in emergency medicine at the University of Wisconsin Hospital and Clinics. Dr. Van Vliet’s practice philosophy and commitment to emergency medicine is driven by a deep-seated belief in human rights, which was nurtured during her time in the Fellowships at Auschwitz for the Study of Professional Ethics program in medical school.
