Making The Health Care Social Service Link: The CareSource Life Services Case Study

The issue of addressing the social determinants of health (SDH) includes concern over a variety of socioeconomic variables, such as economic stability, housing, nutrition, and neighborhood context. But key to all of these is often employment, and finding ways to address consumer employment challenges is gaining increased traction from health plans and providers alike.
