Home-Based Mental Health Evaluation (HOME) Program Raised Veteran Post-Psychiatric Discharge Engagement Rates By 33%
The Home-Based Mental Health Evaluation (HOME) Program raised treatment engagement rates for veterans with post-psychiatric discharges by 17%, compared to those participating in enhanced care as usual (E-CARE). Treatment engagement was defined as attendance at two outpatient mental health appointments in the 90 days following hospital discharge.
The HOME Program supports veteran health care consumers after they are discharged from a psychiatric inpatient unit, offering telephone-based outreach and a home visit until the patient is engaged in care in the 90 days posthospitalization.
